If you are passionate about patient care and thrive in a supportive setting, Health Link is the place for you.Compensation & BenefitsCompetitive Pay: $40.00 – $60.00 per hourUnmatched Field Support: Our experienced clinical team is always ready to assist—whether you need help navigating a complex situation or just need someone to call a patient if you're running late.Flexible Schedule: Take control of your work-life balance.Paid Sick Time: Because your health matters, too.Mileage Reimbursement: Travel expenses for your field visits are covered.Key ResponsibilitiesPerform initial evaluations during the admission process to assess the patient's psychosocial status.Identify socioeconomic and emotional factors that may impact the patient's treatment plan.Collaborate with the interdisciplinary team to develop and implement comprehensive patient care plans.Consult directly with attending physicians regarding any necessary changes to treatment.Maintain and submit accurate, timely clinical documentation (evaluations, care plans, progress notes).Provide monthly supervision to the social work assistant.RequirementsCurrent Licensed Clinical Social Worker (LCSW) credential in California.Valid California driver's license and reliable transportation.Current First Aid and CPR certification.Coverage Area: South Bay and surrounding communities.
