Position Overview

Revive Bone Physical Therapy is seeking a dedicated and experienced Physical Therapist to join our team on a full-time basis. In this role, you will play an essential part in assessing, diagnosing, and treating patients who face physical limitations and injuries. Using your expertise in movement science, you will create individualized rehabilitation programs aimed at restoring function, alleviating pain, and enhancing mobility for patients in their homes.

Work Environment

This position is onsite, which means you will provide care in the patients' own homes. Our home care model allows for a more personalized interaction where you can observe patients in their natural environment, optimizing treatment plans based on real-life scenarios. A supportive administrative team is available to assist you with scheduling and logistics, ensuring that you can focus entirely on patient care.

Key Responsibilities

• Conduct thorough evaluations of patients to determine their physical abilities, limitations, and rehabilitation needs.
• Devise and implement detailed treatment and care plans tailored to each patient's needs and goals.
• Administer hands-on therapy, employing manual therapy techniques, therapeutic exercises, and various modalities to facilitate recovery.
• Utilize independent judgment to adapt treatment strategies based on patient responses and progress.
• Maintain active communication with patients, their families, and any other healthcare providers involved in the patient's care, addressing concerns and updating them on progress.
• Document all treatment interventions and patient progress accurately in compliance with healthcare regulations and organizational policies.
• Educate patients and their families about proper techniques for exercises, safety protocols, and lifestyle modifications to promote long-term health and wellness.

Patient Population / Client Focus

This position involves working with a diverse patient population, including individuals recovering from orthopedic surgeries, athletes rehabilitating from injuries, and older adults managing chronic conditions. Your role will entail designing effective treatment plans to assist each patient in achieving their specific health goals, improving their functional abilities and overall quality of life.

Qualifications

• Bachelor's or Master's degree in Physical Therapy from an accredited program.
• Current and valid Physical Therapist license in the state of Florida.
• A minimum of three years of experience in physical therapy or a related field, with a strong understanding of rehabilitation principles.
• Proficient in using various physical therapy tools and techniques, with a commitment to evidence-based practice.
• Excellent interpersonal skills, enabling effective collaboration with patients and healthcare team members.

Preferred Experience

Experience in home health care settings or working with geriatric populations is highly desirable. Additionally, familiarity with electronic health records (EHR) and digital documentation methods will enhance your capability to deliver effective patient care.

Compensation and Benefits
