Experienced Home Care Physical Therapist (Full-Time)

Work Environment

This position is an onsite role primarily focused on home care, allowing you to work in the comfort of patients' homes and providing personalized treatment. Key Responsibilities

• Conduct comprehensive evaluations of patients in their homes to assess their physical capabilities and limitations.
• Develop and implement individualized treatment plans tailored to each patient's specific medical conditions and rehabilitation goals.
• Utilize a variety of treatment techniques, including therapeutic exercises, manual therapy, and modalities to alleviate pain and enhance mobility.
• Document all treatment interventions, patient responses, and progress rigorously to meet compliance standards and inform ongoing care.
• Reassess and adjust treatment plans based on patient feedback and progress to ensure continuous improvement.
• Provide education to patients, family members, and caregivers about therapeutic techniques and the use of assistive devices, ensuring a supportive home environment.
• Stay current with best practices by attending professional workshops and conferences as well as engaging with professional networks.

Patient Population / Client Focus

You will work with a diverse patient population ranging from children to adults, each with unique therapeutic needs stemming from various injuries, chronic conditions, or post-surgical recovery. Your role will be crucial in helping patients regain their independence and enhance their quality of life through effective rehabilitation strategies.

Qualifications

• A Bachelor's Degree in Physical Therapy is required, with a Doctorate of Physical Therapy (DPT) preferred.
• Must possess a current State License in Physical Therapy and stay updated with all relevant certifications, including a CPR Certificate.
• Excellent verbal and written communication skills to effectively interact with patients, families, and healthcare professionals.
• Proficient in using word processing and basic computer applications for documentation and communication purposes.
• Strong interpersonal skills and the ability to work independently as well as collaboratively within a team environment.

Preferred Experience

• Two (2) years of experience in a related field is required, preferably in a home care setting.
• Experience in educating patients and families in therapeutic techniques and recovery strategies.
• Demonstrated flexibility in adjusting techniques and approaches to meet the diverse needs of patients.

Compensation and Benefits
