Home Health Physical Therapist

Role Overview: The Home Health Physical Therapist provides personalized in-home physical therapy to help patients improve strength, mobility, and independence. Reporting to the Clinical Manager, they assess patient physical strengths, develop and implement rehabilitation programs aligned with physician plans, and document progress and interventions. Responsibilities Plan and deliver prescribed physical therapy treatments in patients' homes to maximize physical function. Assess, document, and support physician evaluations and treatment goals. Oversee and provide clear instructions to PTAs and Home Health Aides. Maintain thorough documentation of patient observations, interventions, and progress. Report patient progress to the healthcare team and complete all required documentation. Qualifications & Skills Current, unrestricted Physical Therapy license. CPR certification. Experience collaborating with healthcare professionals. Preferred: At least one year of clinical experience as a Physical Therapist. Key Requirements: This patient-facing role requires TB screening and a valid driver’s license with personal vehicle insurance.
