Key Responsibilities\n

The primary duties of the Licensed Mental Health Counselor include:

• Conducting thorough reviews of patient health histories to prepare for initial evaluations.

• Developing, implementing, and periodically reevaluating individualized mental health care plans based on established best practices.

• Conducting in-depth interviews with patients and their families to identify educational needs, establish treatment goals, and create tailored plans of care.

• Assessing patient and family responses to education and support, adjusting treatment plans as necessary to enhance effectiveness.

• Collaborating with interdisciplinary team members, including physicians and mental health professionals, to coordinate comprehensive care and maintain clear communication regarding patient status.

• Making appropriate referrals to additional healthcare specialists when needed to ensure holistic treatment.

• Ensuring patient confidentiality in accordance with HIPAA regulations and maintaining proper documentation in our medical record system.

• Facilitating multiple healthy minds workshops designed to educate patients on mental health topics and encourage engagement with their own care.

This position primarily focuses on providing mental health services to adults, including those managing chronic diseases such as cardiac conditions. The successful candidate will have the opportunity to engage with diverse patient profiles, helping them navigate their mental health challenges in connection with their overall health.

Qualifications\n

The ideal candidate will possess the following qualifications:

• A valid mental health professional license (LMHC, LCSW, LMFT, or psychologist).

Compensation and Benefits\n
