A home infusion clinical pharmacist opportunity is available for direct hire in Westerville, OH. This role involves preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications, serving as a key member of a multidisciplinary team focused on patient-centered care.

Location: Westerville, OH
Schedule: Monday – Friday, 8:30 AM to 5:00 PM
On Call: Participation in on-call rotation required
Placement: Direct Hire

Key Responsibilities:

• Prepare specialty pharmaceuticals and IV solutions in a sterile environment, adhering to all state and federal regulations
• Assess lab values, conduct pharmacokinetic dosing calculations (e.g., vancomycin), and monitor Total Parenteral Nutrition (TPN) therapy
• Provide thorough patient and caregiver education on infusion pumps, sterile techniques, and catheter care at home
• Collaborate with physicians, home health nurses, and internal teams to coordinate care and prevent hospital readmissions
• Maintain compliance with regulatory standards, including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA
• Manage pharmacy inventory, including refrigerated products, and ensure accurate packing and timely delivery of medications and supplies

Qualifications:

• Minimum 2 years of infusion specialty pharmacy experience
• IVIg experience is a plus
• In-depth knowledge of specialty therapy payer requirements, reimbursement processes, and contract obligations
• Current and active Ohio pharmacist license in good standing
• Familiarity with CareTend system preferred
