Registered Nurse (RN) - Home Health - $75K-100K per year

Job Overview Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health for an ongoing staff position in Holmes, Pennsylvania. This opportunity supports patients in Havertown, PA and surrounding areas. The role requires 40 hours per week, 8‑hour shifts, and involves leading and collaborating to provide management and delivery of patient‑care plans as a home health registered nurse case manager. The position is eligible for a $10,000 first‑year success bonus and monthly clinical‑outcome bonuses after one year. Responsibilities Lead and collaborate with the care team to manage and deliver patient care plans. Provide home health care to patients, conducting visits, charting, and coordinating services. Adhere to critical deadlines for charting and maintain clinical competence. Use tablets, mobile phones, and EMR software for care documentation. Qualifications RN license in the state where you work. Graduate of an approved practical nursing school. At least one year of RN experience with the competence to deliver quality patient care. Valid driver’s license and ability to spend ~20% of your day driving to patient locations. Commitment to consistently meet critical deadlines for charting.
