Key Responsibilities\n

• Conduct thorough assessments to identify the unique mental health needs of each client, tailoring treatment plans accordingly.

• Facilitate open communication with supervisors, other staff members, family members, and guardians to ensure an integrated support plan for individual clients.

• Prepare and maintain a tentative schedule of appointments, home visits, and necessary observations to track client progress and ascertain areas needing improvement.
\n
• Coordinate home visits that comply with observational requirements, documenting findings to inform future interventions.
\n
• Ensure effective implementation of individualized program plans and adjust as needed based on ongoing evaluations of client progress.
\n
• Participate actively in Interdisciplinary Team Meetings and relevant case discussions to foster comprehensive treatment approaches.
\n
• Produce detailed monthly and quarterly progress notes that provide insights into clients' advancements and therapeutic needs.
\n
• Organize training sessions to equip staff and team members with the necessary skills to effectively execute program plans and maintain accurate documentation.
\n
• Provide ongoing support through follow-up interviews and visits, addressing any emerging challenges faced by clients.
\n
• Act as a liaison within the public and mental health community, contributing to the development of service goals and outreach initiatives.

• Assist in crafting and updating training materials aimed at enhancing programs for individuals.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is mandatory.
