Key Responsibilities\n

• Conduct thorough assessments to understand and meet the unique needs of each patient, ensuring that appropriate treatment plans are in place.

• Maintain excellent communication with supervisors, direct care staff, and family members regarding each patient's progress and any necessary adjustments to their treatment plans.

• Provide a tentative schedule to your supervisor for appointments, home visits, and observations to ensure that all aspects of patient care are monitored effectively.

• Coordinate home visits to conduct in-home observations, chart reviews, and staff interviews, contributing to comprehensive patient evaluations.

• Ensure the effective implementation of individualized program plans and participate in interdisciplinary team meetings that discuss patient care strategies.

• Maintain diligent records to guarantee the readiness of information and documentation for interested parties, facilitating seamless patient care.
\n
• Develop and conduct training sessions for staff members to enhance their competency in implementing program plans and documentation requirements.
\n
• Provide necessary follow-up interviews and home visits to ensure ongoing support for each patient's needs.

• Act as a liaison with the broader mental health community to foster relationships that contribute to the development of effective support strategies for clients.

Patient Population / Client Focus\n

As a Licensed Clinical Professional Counselor, you will have the opportunity to work with a diverse client population, including adults and youth facing a variety of mental health challenges. Qualifications\n

Preferred Experience\n

A background in evidence-based treatments, such as Cognitive Behavioral Therapy (CBT) or Dialectical Behavior Therapy (DBT), along with experience in community mental health settings, will be advantageous. Understanding of trauma-informed care and techniques to engage clients from varied socio-economic backgrounds will also be beneficial.
