Key Responsibilities\n

• Conduct continuous, thorough assessments of patients to customize therapeutic approaches that best meet their individual needs.

• Act as a vital communication link among supervisors, care staff, and family members to keep all parties informed about patient progress and treatment plans.

• Develop and share a structured schedule with supervisors that outlines appointments, home visits, and therapeutic observations to track client advancements effectively.

• Facilitate and coordinate home visits to observe patient behavior in natural settings and gather essential data through chart reviews and staff interviews.

• Ensure that treatment plans are implemented correctly and effectively followed by both staff and patients.
\n
• Participate in interdisciplinary team meetings to discuss patient progress, share insights, and explore collaborative treatment strategies.

• Prepare and document monthly and quarterly progress notes to reflect changes in client status and treatment efficacy.
\n
• Organize and lead training sessions for team members to enhance their understanding of program plans and documentation requirements.

• Provide ongoing support through follow-up interviews and home visits aimed at addressing evolving patient needs.

• Maintain comprehensive and organized records to ensure information is available for all stakeholders involved in the patient's care.

• Act as a liaison with the wider public and mental health community, fostering partnerships that support development goals and expand service outreach.

You will be working with a diverse client population, providing counseling services that cater to varying needs. Key focus areas include, but are not limited to, assisting individuals with trauma, substance misuse, and relationship issues, whether working with families, couples, or individuals. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, Social Work, or a related field is required.

• Possession of active and unrestricted licensure as a Professional Counselor (LPC) in the state of Arizona is mandatory.

• Specialization in areas such as family therapy, couples therapy, substance abuse, or trauma-informed care is highly regarded.

Compensation and Benefits\n
