Key Responsibilities\n

• Work collaboratively with primary care practitioners to identify, assess, and manage mental health issues, contributing to integrated care within our healthcare team.

• Conduct thorough patient intakes and diagnostic evaluations, setting individualized therapy goals and recommendations for treatment in conjunction with primary care providers.

• Deliver immediate mental health interventions for patients in crisis, ensuring timely access to care.

• Make informed recommendations to primary care practitioners regarding suitable mental health referrals and services.
\n
• Perform follow-up evaluations, strategically monitoring treatment efficacy and adjusting care plans in response to patient progress.

• Develop relapse prevention strategies with patients facing mental health challenges, providing ongoing support and guidance.
\n
• Collaborate with a wide range of professionals, including social workers and psychiatric specialists, to enhance the overall delivery of mental healthcare.

• Document all interactions and patient progress thoroughly in compliance with Agency and Federally Qualified Health Center (FQHC) regulations.

• Maintain proactive communication with other team members and primary care providers to ensure continuity of care and a comprehensive understanding of patient needs.

• Empower patients through self-management skills and education while helping them navigate the complexities of their treatment journey.

• Contribute to the design and evaluation of behavioral health programs, ensuring alignment with industry best practices and emerging needs.

• Adhere to the Clinical Professionals' Code of Ethics and participate in ongoing training to further develop your clinical skills.

Qualifications\n

• A Master's Degree in Clinical Counseling is mandatory.
