Key Responsibilities\n

• Conduct comprehensive, ongoing assessments of clients to identify their mental health needs and tailor treatment plans accordingly.

• Foster seamless communication among supervisors, direct care staff, and family members to ensure a cohesive treatment approach and keep all stakeholders informed about a client’s progress.

• Provide supervisors with detailed scheduling of appointments, home visits, and observations to monitor progress and enhance accountability.
\n
• Coordinate in-home observations, conduct chart reviews, participate in staff interviews, and utilize data analysis to fulfill individualized care requirements.
\n
• Guarantee the effective implementation of program plans, actively participating in Interdisciplinary Team Meetings to contribute to collaborative care efforts.
\n
• Generate thorough monthly and quarterly progress notes that document client advancements toward goals.
\n
• Organize and facilitate training for staff to ensure team members are proficient in program plans and documentation practices.
\n
• Conduct follow-up interviews and home visits as needed, ensuring continued support for clients' evolving needs.
\n
• Maintain accurate client records for easy access by authorized personnel and provide transparency in care processes.
\n
• Serve as a representative to the public and mental health community, supporting program development initiatives that meet broader community mental health goals.

Patient Population / Client Focus\n

This role emphasizes working with a diverse range of clients, including both adults and youths experiencing various mental health challenges. You will provide therapeutic support to individuals dealing with issues such as anxiety, depression, trauma, and substance misuse, among others. Qualifications\n

• Master's or Doctoral degree in Counseling, Psychology, or a closely related mental health field.

• Active and unrestricted licensure as a Clinical Professional Counselor (LCPC) in the state of Illinois.

• A minimum of 2 years of experience in counseling, particularly in a clinical setting.

• Strong expertise in family therapy, couples therapy, or trauma-focused modalities is preferred.

• Comprehensive understanding of legal and ethical standards governing clinical practice.

Preferred Experience\n

• Annual salary range of $90,000 to $110,000, commensurate with experience and caseload.
