Work Environment\n

This position is fully remote, leveraging telehealth technologies to reach and service a diverse clientele throughout Chicago, Illinois. As a telehealth specialist, you will conduct virtual sessions and interact with clients primarily through secure digital platforms. Key Responsibilities\n

• Conduct thorough, ongoing assessments to identify and address the unique mental health needs of each patient.
\n
• Maintain clear communication between supervisors, direct care staff, clients, and their families to track and discuss individual progress.
\n
• Create a detailed schedule of appointments and home visits, planning for regular observations that aid in monitoring patient progress.
\n
• Coordinate necessary home visits effectively, which may include chart reviews, interviews, and data analyses to ensure holistic care.
\n
• Implement patient-centered program plans, ensuring each treatment strategy is executed effectively and consistently.
\n
• Actively participate in interdisciplinary team meetings, sharing insights and collaborating with other professionals to enhance patient care.
\n
• Prepare monthly and quarterly progress notes that document patient advancements and necessary adjustments to treatment plans.
\n
• Organize training sessions for staff to ensure competency in program implementation and accurate documentation.
\n
• Provide follow-up interviews and virtual home visits to assess ongoing needs and support the wellbeing of clients.
\n
• Maintain comprehensive records that ensure information is available to all relevant parties involved in patient care.
\n
• Serve as a liaison with the public and mental health community to support developmental goals and promote mental health awareness.

• Assist in developing training materials and procedures aimed at enhancing program effectiveness for clients.

Patient Population / Client Focus\n

Your work will impact a diverse client base, including adults and youth dealing with a range of mental health challenges. You will have the opportunity to provide individualized care while addressing issues related to family dynamics, trauma, substance use, and other critical areas that affect mental wellbeing.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• You must hold an active, unrestricted license as a Clinical Professional Counselor (LCPC) in the state of Illinois.

• A minimum of two years of professional counseling experience is required, with a strong preference for experience in clinical environments.
\n
• Specializations in areas such as family therapy, couples therapy, trauma-informed care, or substance misuse treatment are highly desired.

• Strong understanding of legal and ethical standards relevant to mental health practice.
\n
• Exceptional interpersonal and communication skills to effectively connect and build rapport with diverse clientele.

• Proven ability to collaborate with interdisciplinary healthcare teams.
\n
• Familiarity with Electronic Medical Record (EMR) systems and other relevant healthcare technologies is essential.

Compensation and Benefits\n
