Key Responsibilities\n

• Conduct ongoing clinical assessments to identify patient needs and establish appropriate treatment plans.

• Effectively communicate individual progress to a multidisciplinary team, including supervisors, support staff, family members, and guardians, ensuring all stakeholders are informed and engaged.

• Organize and maintain a schedule of appointments, home visits, and follow-up observations to track patient advancement and ensure continuity of care.

• Facilitate in-home visits where necessary, conducting observations, chart reviews, and staff interviews to collect comprehensive patient data.

• Participate actively in Interdisciplinary Team Meetings, sharing insights and recommendations to enhance patient care strategies.
\n
• Document monthly and quarterly progress notes, summarizing client developments and adjusting treatment plans based on evolving needs.
\n
• Lead training sessions for new staff members and team members on implementation of treatment plans and documentation, ensuring compliance with organizational standards.
\n
• Act as a liaison for clients within the mental health community, providing support and guidance towards their development goals.
\n
• Assist in creating procedures for training materials aimed at improving the effectiveness of counseling programs.
\n
• Provide ongoing follow-up interviews and home visits as required to ensure continued support for clients.

In this role, you will work with a diverse clientele, including both adults and youth facing various mental health challenges. Qualifications\n

• Master's or Doctoral degree in Counseling, Psychology, or a related field is required.

• Active and unrestricted licensure as a Clinical Professional Counselor (LCPC) in the state of Illinois.
