Description: The unit accommodates 100 residents with 96 semi-private beds and 4 private beds across 2 floors. There are no locked units; patients with dementia and Alzheimer's wear trackers that lock doors if they approach them. The facility uses an electronic medical record system in conjunction with paper charting. Patient-to-staff ratios are 1:17-20. If assigned as a House Supervisor, role will include overseeing the entire facility. Common diagnoses among patients include diabetes, COPD, post-stroke conditions, heart disease, fractures, dementia, Alzheimer's, rehabilitation needs, chronic conditions, and multiple comorbidities. Duties include medication administration, vital sign checks, blood sugar management, wound care, ostomy management, foley management, tube feeding administration, and supporting team members. Occasional management of IV antibiotics and potential PICC line management required. No telemetry monitors are present in the facility. Registered Nurses are required to attend daily huddles for team meetings. A Supervisor is available on every shift for support as needed. Nurse Practitioners conduct rounds each morning on weekdays, while two physicians are available in the facility at specified times each week. On-call providers can be reached from the hospital after hours. Interdisciplinary teams include physical therapists, occupational therapists, speech-language pathologists, case managers, social workers, and dieticians. Respiratory therapy is not available on-site 24/7; LPNs and RNs are expected to manage respiratory treatments and equipment. Medications are dispensed in bubble packs and there is a stock of generic medications available. Staffing ratios typically consist of 3 RNs and/or LPNs along with 4-5 nursing assistants per floor during day shifts. Evening and night shifts typically involve 2 RNs and/or LPNs along with 4-5 nursing assistants per floor. Schedules are made six weeks in advance, and travelers must remain flexible to fill staffing gaps. Weekend shifts are required every other weekend, and holiday work may also be necessary. No on-call or standby requirements exist; permanent staff cover these duties. Floating between both floors may be required. Professional scrub attire is necessary; no big earrings, jewelry, or fake nails are permitted, and hair must be tied up. Face tattoos are not allowed.
