Licensed Practical Nurse (Per Diem)

Job Summary As an LPN at NCHHHA you will get to know individual patients and support team members in executing tailored plans of care for Home Health and Hospice patients. You will perform comprehensive assessments and provide skilled nursing care for each patient under the supervision of a qualified Registered Nurse. LPNs are integral to a dynamic, multi‑disciplinary team, maintaining a high level of autonomy and working at the top of your license. You will enjoy a flexible workday with the option to start and end your day from home. Strong clinical support and educational resources are provided, including assistance in obtaining certifications in your area of expertise. Essential Qualifications Must have a current active NH LPN License. Minimum of 1 year experience in a healthcare setting. Ability to demonstrate flexibility in response to unexpected changes in work volume and schedule. Ability to function in any home situation regardless of the patient’s age, race, creed, color, sex, disability or financial condition. Must possess a disposition and personality to work harmoniously with a wide variety of people and all levels of staff and to interact professionally as a member of a multi‑disciplinary team. Speak, write, read and comprehend English. Computer proficiency in electronic medical records data entry. Maintains a valid driver’s license and certificate of auto insurance coverage. Current BLS Certification required.
