Key Responsibilities\n

Your daily tasks will include:

• Conducting comprehensive assessments to tailor treatment plans that meet the unique needs of each patient.

• Facilitating communication between supervisors, family members, and relevant stakeholders to actively track an individual’s progress toward therapeutic goals.

• Providing a structured schedule for appointments and home visits to monitor and evaluate the progress of clients periodically.

• Coordinating necessary in-home visits for assessments, observations, and data analysis while maintaining strict confidentiality.

• Ensuring the effective implementation of treatment plans and participating in regular interdisciplinary team meetings to review case statuses.

• Documenting monthly and quarterly progress notes to keep precise records of each client's journey.
\n
• Organizing and delivering training sessions to ensure all team members are skilled in the program protocols and documentation requirements.

• Engaging in follow-up assessments to ensure continuity of care and support for individual needs.

• Maintaining comprehensive records and acting as a liaison to the public and mental health community to promote service awareness and development.

Qualifications\n

To be considered for this role, candidates must possess:
