Work Environment\n

This position is entirely remote, allowing for flexible work arrangements while still providing essential care through a community health clinic model. As a Social Worker at Bliss Reformation and Wellness Center, you will operate within a structured schedule of Monday to Friday, 8:00 AM to 4:30 PM, with the willingness to adapt to occasional evening and weekend hours based on client needs. This role requires flexibility in scheduling to ensure consistent engagement and support for clients, as well as availability for on-call situations when immediate intervention may be necessary.

Key Responsibilities\n

In this role, you will:

• Conduct comprehensive assessments to understand each client's psychosocial needs, factoring in emotional, mental, and social influences.

• Develop and implement personalized care plans in collaboration with clients and their families, focusing on their unique goals and preferences.

• Provide counseling and emotional support during critical life transitions, helping clients navigate complex challenges with empathy and understanding.

• Advocate for clients’ rights, ensuring access to community resources and social services, thereby enhancing individualized support networks.

• Engage in interdisciplinary collaboration, working with nurses, caregivers, and other professionals to coordinate holistic care for clients.

• Regularly monitor client well-being through scheduled visits, addressing any immediate concerns or potential safety issues in their living conditions.

• Facilitate effective communication and conflict resolution among family members involved in the care of senior clients.

• Maintain meticulous records of client interactions, assessments, progress notes, and care plans in compliance with ethical standards and regulations.

Qualifications\n

To qualify for this position, candidates must possess:
