Key Responsibilities\n

• Conduct thorough clinical assessments and develop comprehensive treatment plans tailored to the unique needs of each patient, utilizing a range of evidence-based therapeutic techniques.

• Implement therapeutic interventions including cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), providing personalized care that meets the psychosocial needs of patients in hospice.

• Document clinical notes accurately and in compliance with HIPAA regulations, ensuring all patient information is maintained confidentially.

• Collaborate with a multidisciplinary team, which may include psychiatrists, psychologists, and nurses, to coordinate holistic care plans, fostering effective communication and teamwork.

• Participate in case consultations and peer review meetings that aim to continuously improve the standard of care being provided to patients.

Patient Population / Client Focus\n

In this role, you will work primarily with patients in hospice care, addressing a multitude of psychological and emotional challenges that arise during serious and life-limiting illnesses. Qualifications\n

• A Master’s degree in Social Work from an accredited institution is mandatory.

• A valid license to practice as a Licensed Clinical Social Worker (LCSW) in California.

• A minimum of two years of clinical experience working in a mental health setting, particularly with patients experiencing chronic or terminal illnesses is preferred.
