Job Summary:

Position Title: Licensed Practical Nurse (LPN) - LTC
Location: Rutland, VT 05702
Duration: 13-week contract with possible extension
Work Schedule: Evenings (2:00 PM - 10:30 PM)
Guaranteed Hours: 40 Hours/Week
Weekend Requirement: Weekends Required
Pay rate: 45$/hr.

Responsibilities for this position include, but are not limited to:

• Administer medications and treatments as ordered by the physician
• Monitor residents' conditions and promptly report any changes to the RN or physician
• Provide direct nursing care, including wound care and other clinical procedures
• Maintain accurate and timely patient records
• Assist residents with activities of daily living (ADLs) as needed
• Collaborate with RNs, CNAs, physicians, and other healthcare team members to provide quality resident care
• Follow infection prevention, safety protocols, and facility policies
• Deliver compassionate, resident-centered care while promoting dignity, comfort, and independence

To be considered for this position, you should have: [Skills, Education, or Experience]

• Minimum 2 years of Long-Term Care (LTC) experience preferred
• Skilled Nursing Facility (SNF) experience preferred
• Proficient in medication administration and nursing documentation
• Strong assessment, communication, and patient care skills
• Ability to work effectively in a fast-paced healthcare environment
• Graduate of an accredited Practical Nursing (LPN) program
• Active Vermont LPN License (or Compact License, if applicable)
• BLS Certification required
