Key Responsibilities\n

• Conduct psychosocial assessments and formulate appropriate treatment plans tailored to the unique needs of each patient.

• Implement evidenced-based behavioral therapies including solution-focused therapy, motivational interviewing, and problem-solving treatment to foster positive patient outcomes.

• Collaborate closely with interdisciplinary healthcare teams, including doctors and psychologists, to provide integrated and comprehensive care.

• Monitor patient responses to therapy, documenting progress and making necessary adjustments to treatment strategies as required.

• Assist patients in navigating their care journey, focusing on adherence to prescribed psychotropic medications and addressing any side effects or challenges faced during treatment.

• Engage in regular consultations and updates with care providers to ensure alignment on treatment objectives and progress.

Patient Population / Client Focus\n

As an LCSW within our organization, you will work predominantly with patients experiencing significant emotional and psychological challenges associated with illness and end-of-life transitions in a hospice setting. Your expertise will play a crucial role in supporting these individuals and their families as they navigate complex emotions and difficult decisions. You will be instrumental in ensuring that patients receive compassionate and comprehensive care tailored to their specific circumstances.

Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• A current, valid, and unrestricted license to practice independently as a Licensed Clinical Social Worker (LCSW) in the State of California.
