Key Responsibilities

• Conduct thorough initial evaluations, which include reviewing patient history, performing physical assessments, and documenting findings to develop a customized treatment plan.
• Communicate assessment results and treatment plans clearly to patients and their families, ensuring their active participation in goal setting and decision-making processes.
• Implement evidence-based treatment protocols, adapting interventions based on each patient's progress and response to therapy in a dynamic healthcare environment.
• Collaborate with other healthcare professionals to provide holistic care, recognizing shifts in patient's health status and adjusting care plans as necessary.
• Manage patient discharge planning efficiently, ensuring a seamless transition to home or community care and conducting necessary follow-up assessments.
• Maintain accurate and timely documentation of patient care activities, including evaluations, treatment plans, encounter notes, and discharge summaries to meet regulatory standards.
• Adhere to established productivity benchmarks while effectively managing your time and scheduling.
• Engage in marketing initiatives to promote our services within the community and foster relationships that enhance our reach and patient access.

Patient Population / Client Focus

You will primarily work with our diverse patient population, which includes individuals recovering from orthopedic surgeries, sports injuries, neurological disorders, and chronic pain conditions. Your goal will be to empower them to restore function, increase mobility, and improve their overall quality of life. The alignment of our treatment strategies with patient-centered care ensures positive outcomes and significant improvements in their capabilities.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) is required.
• Active Physical Therapy License to practice in Virginia.
• Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association is mandatory.
• While previous work experience is valued, it is not a requisite for this role; recent graduates are encouraged to apply.

Preferred Experience

• A minimum of two years of clinical experience in an outpatient rehabilitation setting is preferred, showcasing a strong ability to work independently and as part of a multidisciplinary team.
• Experience in treating a varied clientele, including athletes, post-surgical patients, and individuals with chronic pain conditions, would be beneficial.
• Familiarity with electronic health record (EHR) systems and experience in patient documentation processes.

Compensation and Benefits
