Key Responsibilities\n

• Conduct comprehensive clinical assessments and develop tailored treatment plans that align with the needs of patients experiencing end-of-life challenges.

• Utilize solution-focused therapy techniques to provide therapeutic interventions addressing grief, loss, and other emotional issues faced by patients and their families.

• Work collaboratively with the interdisciplinary team to ensure a holistic approach to patient care, facilitating effective communication and coordinated services.

• Lead case consultations and peer review sessions, sharing insights and strategies to continuously elevate the standard of care.

• Educate patients and families about available mental health resources, treatment options, and coping mechanisms, empowering them in the decision-making process during this challenging time.

• Engage in ongoing professional development by staying current with industry research, best practices, and innovations in palliative and hospice care.

Patient Population / Client Focus\n

This role specifically focuses on hospice patients, their families, and caregivers who require compassionate support during end-of-life transitions. Qualifications\n

• A Master’s degree in Social Work from a program accredited by the Council on Social Work Education (CSWE).

• An active Licensed Independent Clinical Social Worker (LICSW) license in the state of Washington.

• A minimum of two years of experience in clinical social work, preferably in a healthcare or hospice setting.

Compensation and Benefits\n
