Key Responsibilities\n

• Conduct thorough assessments to identify the social, emotional, and environmental challenges faced by clients, ensuring a tailored approach to each individual’s situation.

• Provide individual and family counseling to assist clients in overcoming life challenges related to health, mental well-being, or socio-economic factors.

• Collaborate closely with clients to create personalized service plans and integrate resources from our extensive network of community organizations and agencies.

• Offer critical crisis intervention services, conduct risk assessments, and engage in safety planning to support clients during urgent times.

• Actively coordinate with various community agencies to ensure comprehensive service delivery and strong advocacy for client needs.

• Engage in proactive community outreach to bolster awareness of agency services, enhancing engagement and partnerships with local resources.

• Participate in regular staff meetings, case conferences, and training sessions to foster professional development and teamwork.

Patient Population / Client Focus\n

The LCSW will work with a diverse client population, including individuals and families facing life-limiting illnesses and the unique psychosocial challenges these bring. Qualifications\n

Compensation and Benefits\n

The annual salary for this position ranges from USD 60,000 to 100,000, commensurate with experience and qualifications. In addition to competitive pay, we offer a sign-on bonus for new hires.
