Key Responsibilities\n

• Engage actively with the care team to establish and maintain effective communication channels, enabling referrals and the development of integrated care plans.

• Conduct thorough assessments through telephonic evaluations and document findings efficiently in compliance with established timelines and electronic health records.

• Regularly monitor and review treatment plans to ensure their relevance and effectiveness, making amendments as needed based on progress and evolving client needs.

• Collaborate with the multidisciplinary care team to enhance identified patient outcomes, ensuring that care is patient-centered and culturally competent.

• Coordinate treatment plans that incorporate community resources when appropriate, thereby expanding the support network available to clients.

• Utilize our electronic medical/health system for the complete documentation of patient interactions, safeguarding compliance and accuracy.

Patient Population / Client Focus\n

Our services cater to a diverse clientele, including individuals, couples, and families dealing with various mental health challenges. You will work with clients facing anxiety, depression, relationship issues, and trauma, among others. The primary aim is to foster resilience and empower our clients to navigate their circumstances positively. By addressing patients' psychological and social needs, your work as an LMFT will directly influence their quality of life and family dynamics.

Qualifications\n

• A Master’s degree in Marriage and Family Therapy from an accredited institution.

• Current and active licensure as a Licensed Marriage and Family Therapist (LMFT) in California.

• A minimum of three years of experience in an outpatient mental health setting, with a proven ability to address a range of issues effectively.

• Strong proficiency in electronic health record systems, along with a solid foundation in Microsoft Office applications, including word processing and spreadsheets.

Compensation and Benefits\n
