Work Environment\n

This role is primarily remote, allowing for flexible work arrangements that support your work-life balance. You will engage with patients through telehealth platforms, ensuring they receive the necessary support and guidance without geographical constraints. Key Responsibilities\n

• Conduct comprehensive, age, and culturally appropriate bio-psychosocial assessments for patients experiencing mental health issues, substance abuse challenges, and other medical needs within a primary care framework.

• Provide a range of essential behavioral health services, including but not limited to individual and group therapy, crisis intervention, diagnosis, treatment planning, and referrals to substance use treatment programs when necessary.
\n
• Facilitate communication between patients and various healthcare providers, ensuring that patient needs are accurately documented and escalated when urgent care is indicated.
\n
• Maintain up-to-date electronic health records by documenting evaluations, treatment plans, progress notes, and other critical information in accordance with state regulations and organizational policies.
\n
• Collaborate with Quality Improvement teams to develop, execute, and assess data collection processes that allow for the enhancement of service delivery and patient satisfaction.
\n
• Act as a liaison by providing social work support and connecting clients to local community resources, including medication assistance programs, to address their unique needs.

You will primarily work with a diverse patient population, including individuals facing mental health challenges, families in need of support, and those who require assistance with substance use. Qualifications\n

• Current and valid LCSW licensure in the state of California is mandatory.

Compensation and Benefits\n
