This position primarily operates in a remote setting while also requiring occasional visits to our Hospice Center in Fall River, MA, for team meetings and community outreach. The traditional work hours are Monday through Friday, from 8:00 AM to 4:30 PM, though flexibility with schedule may be needed to accommodate client needs, including occasional evenings or weekends. Key Responsibilities\n

• Conduct comprehensive assessments of seniors’ emotional, mental, and social needs, using evidence-based methodologies to inform care decisions.

• Provide counseling and emotional support, helping clients navigate complex life transitions, including end-of-life considerations.

• Maintain thorough and accurate records of client assessments, care plans, progress notes, and any relevant interactions in compliance with organizational policies.

This role will primarily focus on working with elderly clients in hospice care, many of whom face complex emotional and physical challenges related to serious health conditions. Qualifications\n

Compensation and Benefits\n
