Key Responsibilities

• Conduct comprehensive psychosocial assessments to evaluate patients' mental health status, focusing on their emotional, social, and psychological needs.
• Administer evidence-based therapeutic interventions such as Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT) tailored to individual patient circumstances.
• Collaborate closely with a multidisciplinary team, including psychiatrists and clinical staff, to coordinate care plans and treatment strategies.
• Empower patients and families by helping them understand diagnoses, treatment options, and medication regimens.
• Facilitate individual and group therapy sessions that provide a safe space for emotional expression and healing.
• Create, implement, and regularly update personalized treatment plans in partnership with patients, considering their goals and preferences.
• Guide patients through the complex landscape of the mental health system by advocating for their rights and addressing their unique needs effectively.
• Lead therapeutic groups and educational sessions to equip patients and families with coping strategies during the hospice care process.

Patient Population / Client Focus

Qualifications

• A Master’s degree in Social Work from an accredited institution.
• Possession of an active state license as a Licensed Clinical Social Worker (LCSW).
