Key Responsibilities

• Conduct thorough psychosocial assessments through virtual interviews, observations, and a comprehensive review of patients' medical histories to identify emotional, social, and environmental factors impacting their care.
• Provide empathetic counseling and support to patients and their families, addressing various adjustment issues, emotional struggles, and overall psychological well-being throughout the hospice process.
• Collaborate closely with the hospice care team to develop individualized treatment plans that incorporate psychosocial needs and support systems, tailored to each unique patient situation.
• Assist patients in accessing critical community resources, including support groups, vocational rehabilitation, and housing assistance, contributing to a more streamlined care experience.
• Facilitate discharge planning and coordinate transitions to other care settings, enabling a smooth transfer of care that considers all facets of the patients' needs.
• Conduct regular follow-ups with families post-transition to provide ongoing support and address any arising concerns.

Patient Population / Client Focus

Your role will involve assisting patients with terminal illnesses and their families, focusing particularly on enhancing quality of life, managing symptoms, and addressing emotional trauma associated with end-of-life decisions. Qualifications

• A Master’s degree in Social Work from an accredited program is required, ensuring a solid theoretical foundation for practical application in the field.
• Must possess an active state license as a Licensed Clinical Social Worker (LCSW), compliant with current state requirements.
• Demonstrated understanding of hospice care principles, including adjustment issues related to grief, loss, and terminal illnesses, is essential.
• Excellent communication and interpersonal skills are vital, with an ability to build rapport quickly with patients and families under stress.

Preferred Experience

• Experience in hospice, palliative care, or similar health care settings where social work principles are applied is preferred.

Compensation and Benefits
