This position offers the flexibility of remote work, allowing you to deliver care from your home while maintaining direct communication with clients, caregivers, and healthcare professionals. Although you will primarily engage with clients virtually, occasional site visits to the hospice center may be required to ensure comprehensive support and care. Key Responsibilities\n

• Conduct thorough assessments for each client, developing tailored treatment plans that address their unique needs and circumstances.

• Engage in individual, group, and family counseling sessions, employing therapeutic techniques that are particularly effective in hospice settings.

• Monitor clients' progress towards their goals, documenting interactions and progress notes in our electronic health record (EHR) system.

• Collaborate with a multidisciplinary team of healthcare professionals, including physicians, nurses, and therapists, to create a cohesive, integrated care plan for each patient.

• Provide crisis intervention services when necessary, ensuring that clients are supported during high-stress situations.

• Stay informed about the latest research, methodologies, and regulatory requirements in the field of clinical social work and hospice care.

Patient Population / Client Focus\n

As a Licensed Clinical Social Worker at Gentle Guidance, you will primarily work with terminally ill patients and their families facing emotional distress, anxiety, and grief. You will adeptly navigate conversations about end-of-life concerns, helping clients process their feelings and adjust to changes in their healthcare narrative. By offering empathetic support, you will be instrumental in enhancing the emotional and psychological well-being of both patients and their loved ones during a vulnerable phase of life.

Qualifications\n

• Master's degree in Social Work from an accredited institution, with a focus on clinical practice.

• A valid Kansas state license to provide clinical social work services.
