Key Responsibilities\n

• Develop and periodically revise individualized treatment plans based on comprehensive psychosocial assessments that evaluate patients’ and families’ concerns, strengths, needs, and values, including spiritual and cultural considerations.

• Facilitate educational and therapeutic group sessions to provide psychoeducation on coping strategies, emotional processing, and resources, while documenting patient progress and responses.

• Actively participate in interdisciplinary treatment planning meetings to exchange feedback, assess patient progress, address any resistance, and update treatment plans as necessary.

• Establish strategic alliances with local community services to ensure thorough aftercare arrangements are in place, facilitating a smooth transition for patients post-service.

• Maintain meticulous records and documentation of all patient interactions, assessments, and treatment plans to ensure compliance with regulatory standards and enhance treatment quality.

• Provide emotional support, crisis intervention, and guidance to patients and their families, fostering a safe environment for open communication and emotional expression.

Patient Population / Client Focus\n

In this role, you will work primarily with patients facing life-limiting conditions in a hospice center. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Active licensure as a Clinical Social Worker (LCSW) in California.

Compensation and Benefits\n

This position offers a competitive salary ranging from $120,000 to $130,000 annually, commensurate with experience and caseload.
