Key Responsibilities\n

As a Licensed Clinical Social Worker, your primary responsibilities will include:

• Conducting comprehensive psychosocial assessments to evaluate patients' emotional, social, and financial needs, aiding them in navigating their treatment journey.

• Creating and implementing individualized treatment plans in collaboration with our interdisciplinary medical teams, focusing on holistic care.

• Providing individual and group counseling focused on issues such as adjustment, grief, and coping mechanisms, utilizing evidence-based therapeutic techniques.

• Connecting patients with community resources and services to ensure continuity of care following discharge and facilitate smooth transitions.

• Offering emotional support and education to patients and their families, arming them with essential knowledge regarding treatment options and community services.

• Participating actively in hospital committees that address social work-related matters, contributing to policy development and practice improvement.

• Documenting patient interactions and treatment plans accurately and promptly, ensuring compliance with hospital policies and legal standards.

• Providing crisis intervention and supportive counseling during acute situations, maintaining a calm and professional demeanor.

Patient Population / Client Focus\n

Your work will primarily focus on supporting patients undergoing detoxification and recovery from various substance use disorders, alongside those dealing with associated mental health challenges. You will liaise with individuals facing complex situations, including emotional distress, social isolation, and additional co-occurring disorders. Your expertise will be crucial in offering guidance and developing personalized care plans that address the unique needs of each patient.

Qualifications\n

The ideal candidate will possess:

• A Master’s degree in Social Work from an accredited institution.

• Current active licensure as a Licensed Clinical Social Worker (LCSW) in the state of California.

Preferred Experience\n

While two years of experience in a healthcare or hospital setting is preferred, we also welcome applicants who demonstrate relevant volunteer or internship experience in similar environments. Compensation and Benefits\n
