Work Environment\n

The LCSW position is designed to be remote, offering the flexibility to work from home while maintaining a structured schedule. Our standard operating hours are Monday through Friday from 9:00 AM to 5:00 PM. Key Responsibilities\n

• Provide individual and group psychotherapy services to patients coping with mental health disorders, establishing a trusting relationship to create a supportive therapeutic environment.

• Conduct comprehensive assessments of patients’ mental health needs, factoring in emotional, behavioral, and social elements that may impact their well-being.

• Develop and implement individualized treatment plans that are evidence-based and culturally appropriate, aligning with the goals and values of each patient.

• Engage in multidisciplinary collaboration with healthcare professionals, including psychiatrists and primary care providers, to coordinate patient care effectively.

• Regularly monitor and evaluate patient progress, making necessary adjustments to treatment plans in response to patient feedback and evolving needs, ensuring continuous improvement and effectiveness of care.

• Provide education and support to patients and their families regarding mental health issues, treatment options, and available community resources, fostering empowerment and understanding.

• Adhere to ethical standards, maintaining confidentiality and informed consent in all interactions and documentation, thereby ensuring compliance with regulatory requirements.

Patient Population / Client Focus\n

This role primarily involves working with patients enrolled in hospice care who may experience complex emotional and mental health challenges. You will be instrumental in addressing their psychological needs, assisting in coping strategies, and enhancing their quality of life during critical transit stages of their health journeys.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in Illinois.

• Minimum of two years of relevant experience in providing psychotherapy and mental health services.

• Exceptional interpersonal and communication skills, enabling you to build rapport with patients and collaborate proficiently with other healthcare professionals.

• Strong clinical assessment skills with a firm understanding of evidence-based practices and treatment methodologies.

• Proficiency in using electronic health record (EHR) systems to maintain accurate and timely patient information.

Compensation and Benefits\n
