Key Responsibilities\n

• Conduct comprehensive and ongoing assessments to determine patients' needs and tailor treatment plans accordingly.

• Effectively communicate the progress of patients with supervisors, direct care staff, and family members to ensure coordinated care.
\n
• Provide a structured schedule to supervisors that outlines appointments, home visits, and follow-up observations for continuous patient support.
\n
• Coordinate and conduct home visits as needed for holistic assessments, which may include chart reviews and engaging with families and caretakers.
\n
• Ensure that treatment plans are executed effectively and that patients are making progress towards their goals.
\n
• Participate in interdisciplinary team meetings and relevant case discussions, sharing insights to optimize patient outcomes.
\n
• Complete monthly and quarterly progress notes to document patient developments and treatment efficacy.
\n
• Organize and conduct training for staff to enhance competency in program implementation, ensuring adherence to protocols.
\n
• Maintain accurate records to ensure all pertinent information is accessible to authorized personnel and stakeholders.
\n
• Act as a liaison between the organization and the broader mental health community to foster collaborative development initiatives.

• Assist in creating and improving training materials that bolster program efficacy and staff preparedness.

Patient Population / Client Focus\n

You will engage with a diverse range of clients, including adults navigating various life challenges and youths facing emotional or behavioral issues. Your approach will be patient-centered, focusing on families and individuals needing counseling across specialties such as trauma recovery, substance use, and relationship issues. Your expertise will help clients gain insights and strategies to manage their circumstances effectively.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Arizona.

• A minimum of two years of counseling experience, preferably in a clinical or telehealth setting.

• Specialization in family therapy, couples therapy, trauma, or substance misuse is highly desirable.

• Strong knowledge of ethical and legal standards in mental health practice.

• Exceptional interpersonal skills, enabling you to establish rapport with a diverse clientele.

• A proven ability to collaborate effectively with multidisciplinary teams.

• Proficiency in Electronic Medical Record (EMR) systems and other relevant technology.

Preferred Experience\n

While at least two years of counseling experience is mandatory, we prefer candidates with a strong background in trauma-informed care or experience working with marginalized populations. Familiarity with telehealth technology and methodologies will be advantageous in this role.
