Key Responsibilities

• Evaluate patients by conducting comprehensive reviews of their medical histories, performing specific assessments, and formulating treatment recommendations.
• Communicate evaluation findings and create personalized treatment plans in collaboration with patients and their families, involving them actively in setting rehabilitation goals.
• Implement evidence-based treatment protocols, demonstrating proficiency in adapting interventions based on ongoing assessments and the evolving healthcare landscape.
• Consult with other healthcare professionals to ensure coordinated care, monitoring changes in patient conditions that may necessitate adjustments to treatment plans.
• Oversee discharge planning and ensure effective follow-up care; maintain accurate, timely documentation illustrating the patient's journey and therapy progress.
• Demonstrate effective time management, meeting productivity expectations while maintaining an organized caseload and schedule.
• Engage in marketing initiatives to promote the clinic's services within the community, effectively attracting new patients.

Patient Population / Client Focus

As a Physical Therapist at Upstream Life Physical Therapy, you will primarily work with patients recovering from surgeries, injuries, or chronic conditions that necessitate individualized rehabilitative care. Qualifications

• A Bachelor's degree in Physical Therapy is required, and a Doctorate degree (DPT) is preferred.
• A valid physical therapy license in the state of Florida.
• American Heart Association's BLS for Healthcare Providers CPR certification is essential.

Preferred Experience
