Work Environment\n

This position is fully remote, offering flexibility to conduct consultations and therapy sessions from the comfort of your home. As part of a community health clinic, you will be integrating clinical expertise with the convenience of telehealth services, allowing you to reach clients effectively and efficiently. The standard work hours are Monday to Friday from 8:00 AM to 4:30 PM, with occasional evening or weekend duties as client needs dictate. This role will require adaptability in scheduling to ensure comprehensive care and on-call availability to address urgent situations.

Key Responsibilities\n

• Conduct comprehensive assessments of clients' psychosocial needs, utilizing evidence-based tools to evaluate emotional, mental, and social factors affecting overall well-being.

• Collaborate closely with clients and their families to create tailored care plans that align with individual preferences and goals, ensuring an active role in their care process.

• Provide ongoing counseling and emotional support to seniors, navigating life transitions, such as relocation to assisted living or coping with loss and grief.

• Work collaboratively with interdisciplinary teams, including nurses, caregivers, and other healthcare professionals, to enhance the quality of care provided to clients.

• Carry out regular check-ins and virtual visits to monitor clients’ health and safety, identifying any changes that require immediate attention.

• Facilitate effective communication among family members involved in the senior client's care, resolving conflicts and improving coordination of services.

• Educate clients and their families about available community resources, support groups, and programs to promote social engagement and overall quality of life.

• Maintain meticulous documentation of client interactions, progress notes, assessments, and care plans while adhering to HIPAA regulations and ethical standards.

Qualifications\n

• Master's degree in Social Work from an accredited institution is required.

Compensation and Benefits\n
