What You'll Be Doing Provide advanced wound care services and community-based rounding throughout senior living communities. Manage pressure injuries, diabetic wounds, post-surgical wounds, skin integrity concerns, and complex cases. Support chronic disease management, preventative care, and care coordination efforts. Collaborate with physicians, nurses, therapists, caregivers, and facility teams. Help improve outcomes while reducing avoidable hospitalizations and complications. Serve as a trusted clinical partner within the communities you support. Qualifications Active Minnesota Certified Nurse Practitioner (CNP) license (AGNP preferred). Experience in wound care, geriatrics, primary care, post-acute care, skilled nursing, or related settings preferred. Strong clinical judgment and confidence practicing independently.
