This role is designed to be executed remotely, enabling you to provide care from the comfort of your own setting while connecting with patients who are part of our school-based rehabilitation program. While you will primarily work during regular business hours on weekdays, some flexibility for evening and weekend commitments may be required based on patient needs and caseload. Key Responsibilities\n

• Conduct in-depth psychosocial assessments by interviewing patients and their families, observing behaviors, and reviewing medical records to identify emotional, social, and environmental factors that affect the recovery process.

• Provide supportive counseling to patients, helping them navigate emotional adjustments, cope with feelings of grief, and develop effective coping strategies.

• Collaborate with an interdisciplinary rehabilitation team to formulate individualized treatment plans that reflect each patient’s unique psychosocial needs and recovery goals.

• Assist patients in connecting with essential community resources, including support groups, vocational rehabilitation services, and housing assistance, ensuring they receive the comprehensive support required after discharge.

• Educate patients and their families on available community resources and advocate for post-rehabilitation services to facilitate a smooth transition back into daily life.

• Actively participate in discharge planning, playing a vital role in preparing patients for their next steps, whether at home or in another care facility.

Your primary focus will be on a diverse group of patients within a school-based setting, which may include children recovering from injury or illness and their families. By addressing their emotional and social needs, you will help improve their rehabilitation outcomes, encouraging independence and successful integration back into their school and community environments. Qualifications\n

• A master's degree in Social Work from an accredited program is required.

Compensation and Benefits\n
