Job DescriptionUW Medicine Primary Care at South Lake Union has an outstanding opportunity for a Between Visit Care Coordinator (PSS).WORK SCHEDULEFull-time, 100% FTE (40 hours per week)Monday-Friday, 8-5Day ShiftDEPARTMENT DESCRIPTION Located on the corner of Dexter Avenue N. and Republican Street, UW Medicine Primary Care at South Lake Union offers primary care appointments six days a week.The clinic offers a full spectrum of primary care services (including family medicine and integrated behavioral health), obstetrics and gynecology, and sports medicine. Also located within the building, you’ll find designated specialty care from the UW Diabetes Institute and the Karalis Johnson Retina Center .POSITION HIGHLIGHTSConveniently located near public transportationMake a difference for the patients and employees we serveOpportunity to provide career growthPRIMARY JOB RESPONSIBILITIESEnsures timely response by following established network standard work and clinic protocols for telephone encounters, Epic in-basket folders, incoming/outgoing phone calls and responding to patient messages in MyChartManages incoming multi-line phone calls, MyChart messages, paperwork and fax transmissions from patients, pharmacies, and other providers. Routes to other care team members as necessary for follow upActs as a liaison between provider and other facilities including UW Medicine entities and departments, pharmacies, home health agencies, specialists, etc.Reviews open telephone and MyChart encounter report and performs audits. Communicates audit results and trends or patterns pertaining to access to care to clinic leadershipGathers information from patient symptom-based calls and routes to nurse triage as appropriate per guidelinesProvide administrative support to the referral process, such as faxing referrals and scanning referral related paperwork as needed to support Patients Are First standardsAssist with forms completions for providers as needed for patients, insurance companies and employers.
