Work Environment

This role provides the flexibility of a remote work setting, primarily within the supportive framework of our Community Center. While most duties can be conducted virtually, the position does require occasional in-person visits to local clients, ensuring that our social worker can maintain relationships and monitor care plans effectively. Regular working hours are Monday through Friday from 8:00 AM to 4:30 PM, with some flexibility for evening or weekend support as client needs arise. On-call availability may also be necessary to address urgent client issues.

Key Responsibilities

• Conduct comprehensive assessments of seniors' psychosocial needs, incorporating emotional, mental, and social factors.
• Collaborate closely with clients and their families to design individualized care plans that align with their unique goals and preferences.
• Provide counseling focused on emotional and psychological support through life transitions, such as health declines or relocation to residential facilities.
• Advocate for clients, ensuring that they have access to pertinent community resources, including healthcare services, support groups, and financial assistance programs.
• Engage with interdisciplinary teams of healthcare professionals to deliver well-rounded care that addresses clients' physical, emotional, and social needs.
• Perform regular virtual and face-to-face check-ins to monitor clients' well-being, assess their living situations, and address any concerns proactively.
• Facilitate productive communication among family members regarding client care, addressing any potential conflicts or misunderstandings.
• Educate clients and their families about available community resources to foster social connections and enhance overall quality of life.
• Maintain accurate documentation of assessments, care plans, progress notes, and interactions in compliance with state regulations and ethical standards.
• Stay current on best practices, updates in policies, and trends in the field of social work, especially related to aging populations.

Patient Population / Client Focus

The primary focus of this role is on seniors living in Potomac and surrounding areas, who may be dealing with various life challenges stemming from aging, such as isolation, health concerns, or family dynamics. The ideal candidate will recognize the importance of addressing not just the physical health of these clients but also their emotional and social well-being, promoting holistic care strategies that prioritize successful aging.

Qualifications

• Master's degree in Social Work (MSW) from an accredited institution.
• Current and valid licensure as a Licensed Certified Social Worker (LCSW-C) in Maryland.
• A minimum of two years of relevant social work experience, particularly in geriatric settings or community-based services.
• In-depth knowledge of aging-related issues, community services, and social work compliance standards.
• Exceptional problem-solving, assessment, and counseling skills.
• Demonstrated ability to empathize with and connect to seniors and their families.
• Strong communication skills, facilitating collaboration with multidisciplinary teams.
• Proficiency in electronic health record (EHR) systems and relevant technology.

Preferred Experience

• Experience working in community centers or in collaboration with healthcare entities serving geriatric populations.
• Knowledge of local community resources and support networks.
• Experience conducting workshops or support groups aimed at family caregivers or seniors.

Compensation and Benefits

The salary for this position ranges from USD 60,000 to 100,000 annually, commensurate with experience and qualifications.
