Work Environment

This position is primarily onsite and involves traveling to patients' homes within the Princeton area. Key Responsibilities

• Administer direct patient care in alignment with the prescriptions from physicians or other authorized practitioners, ensuring adherence to professional guidelines.
• Conduct thorough evaluations using industry-standard assessment techniques, identifying patient needs and establishing a clear basis for treatment strategies.
• Create and implement effective treatment plans to achieve patients' rehabilitation goals, adjusting as necessary based on patient progress and tolerance levels.
• Monitor and document patient responses during treatment sessions, making real-time modifications to therapies as indicated to optimize outcomes.
• Ensure patient safety by adhering to established emergency protocols and responding to any situation that may arise during therapy sessions.
• Maintain accurate and timely documentation in accordance with company policies and regulatory requirements, generating reports that reflect patient progress and treatment effectiveness.
• Engage in quality improvement initiatives through the review of patient charts and participation in departmental activities focused on enhancing service delivery.
• Facilitate family education sessions and conduct interdisciplinary meetings to collaboratively discuss patient management and progress.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• Must possess and maintain NJ therapy licensure in good standing.
• At least two years of experience in a physical therapy role, preferably within a home care setting.

Compensation and Benefits

We offer a competitive salary package ranging from $90,000 to $100,000 per year, commensurate with experience.
