Home Health - Full-Time - Per Visit, Registered Nurse (RN) 1 month ago Be among the first 25 applicants Home Health - Full-Time - Per Visit - Registered Nurse Highland Park, TX, 75205 $2,500 Sign-on Bonus Overview Our full-time Registered Nurse (RN) will have a variety of duties that support continuity of care and overall company operations. The candidate must maintain 32 productivity points weekly to qualify and participate in the compensated on-call program. The RN will see visits assigned within the scope of practice of their license, including OASIS visits, skilled nursing visits, and supervised visits. They will coordinate care with the interdisciplinary team, patients, families, and referring agencies, and participate in the coordination of care. Responsibilities Case management and supervised visits. Perform skilled and comprehensive assessment of the patient during the initial admission. Assess and chart observations of the patient’s condition at each visit (vital signs). Provide skilled nursing care as needed in accordance with the plan of care (administering medication, cleaning/dressing wounds, assessing healing progress). Provide proper patient education for home health care procedures and strategies. Responsible for maintaining clinical records, including visit notes (completed within 24 hours), comprehensive assessments (including OASIS, completed within 48 hours), medication profile, and timely orders/plans. Maintain confidentiality of patient PHI in accordance with HIPAA. Support patient comfort and, where appropriate, palliative care. Qualifications Current Texas State license as a Registered Nurse. 2+ years experience as an RN in Home Health. Current CPR certification. Reliable transportation. Texas Driver's License and Auto Liability Insurance. Current TB Skin Test. EMR experience (Kinsser preferred).
