Conduct thorough evaluations for patients in both inpatient and outpatient environments, creating tailored treatment strategies
Facilitate physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
Collaborate seamlessly with an interdisciplinary team, including nurses, physicians, case managers, and rehabilitation professionals to optimize patient outcomes
Educate patients and their families about therapeutic goals, transitioning plans, and essential home exercises
Maintain accurate records of evaluations, treatments, and progress per facility and regulatory standards
Engage in interdisciplinary team meetings to provide integrated care
Doctorate in Physical Therapy (DPT) or an equivalent qualification from a recognized institution
State licensure as a Physical Therapist
CPR certification preferred
Additional certifications in specialized areas (e.g., neurological or geriatric focus) are beneficial but not mandatory
Prior experience is strongly favored
Proven ability to treat patients with intricate medical and mobility challenges
Excellent communication and interpersonal skills
Familiarity with digital charting and clinical documentation tools
