Bright Star Counseling is seeking a dedicated Licensed Master Social Worker (LMSW) to join our team in Pueblo, Colorado. This full-time, remote position offers an opportunity to impact seniors' lives significantly. As a Senior Care Specialist, you will leverage your expertise to support the psychosocial needs of elderly clients and their families, contributing to improved emotional and social well-being.

Our organization operates a Detox Center that primarily focuses on guiding individuals through substance use challenges. This role is structured for full-time remote work, allowing you to manage your hours effectively while meeting the needs of our clients. The Detox Center operates Monday to Friday, from 8:00 AM to 4:30 PM, with flexibility required for evening and weekend visits to respond to our clients' varying needs. As a member of our professional team, you will collaborate with interdisciplinary professionals, including nurses and healthcare providers, to ensure comprehensive care delivery.

• Conduct detailed assessments of seniors' psychosocial needs to ensure comprehensive support, considering emotional, mental, and social factors.

• Develop and implement personalized care plans in collaboration with clients and their families, focusing on specific goals and preferences.
\n
• Provide therapeutic counseling to individuals and families navigating transitions, instilling resilience and coping strategies.
\n
• Advocate for client rights, connecting them to necessary community resources and ensuring access to relevant services.
\n
• Monitor client progress through regular visits, assessing living conditions and addressing any emerging concerns or safety issues.
\n
• Facilitate clear communication among family members and caregivers to resolve conflicts and enhance the care experience.
\n
• Educate clients and their families about available resources, support groups, and the systems that bolster their social engagement.

• Maintain meticulous records, including client assessments, care plans, and progress notes, via electronic health record (EHR) systems.

• Stay updated on best practices within the field of social work and comply with relevant policies and ethical standards.

• Master's degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW).

• Strong assessment and counseling skills, with an ability to connect empathetically with vulnerable populations.
