Work Environment\n

This is a full-time remote position, allowing you to work from the comfort of your own home while serving clients at our Hospice Center in Pueblo, Colorado. You will be part of a collaborative virtual team that includes healthcare professionals such as nurses and caregivers. Our flexible work hours are structured around client needs, with a standard schedule of Monday to Friday from 8:00 AM to 4:30 PM, with occasional evening or weekend commitments as required.

Key Responsibilities\n

• Conduct comprehensive assessments of seniors to identify psychosocial needs, incorporating emotional, mental, and social considerations.

• Develop individualized care plans in collaboration with clients and their families, ensuring alignment with their specific goals and preferences.

• Provide counseling and emotional support to seniors and families, helping them navigate complex life transitions, such as end-of-life care.

• Advocate for client rights, ensuring access to community resources, healthcare services, and necessary support.
