Work Environment\n

This position offers a full-time remote work setting, allowing for flexibility while ensuring that high-quality support is delivered to our clients. The social work schedule is primarily Monday to Friday, from 8:00 AM to 4:30 PM, with the possibility of occasional evening or weekend hours, depending on client needs. Flexibility is crucial in this role to accommodate client visits and provide continuous care to those in need. Additionally, on-call availability may be required to quickly respond to any urgent situations or client inquiries outside of regular hours.

Key Responsibilities\n

• Conduct comprehensive assessments of seniors' psychosocial needs, considering emotional, mental, and social factors that could impact their well-being.

• Collaborate with clients and their families to create personalized care plans aimed at supporting their unique goals and preferences.

• Provide counseling and emotional support to seniors, helping them cope with various transitions, including health challenges, loss, and changes in living situations.

• Advocate for clients' rights, ensuring they have access to necessary resources and services in the community.

• Work in partnership with an interdisciplinary team, including healthcare professionals, nurses, and caregivers, to coordinate comprehensive services for clients.

• Conduct regular virtual or in-person visits to monitor client welfare, assess living conditions, and address safety or health concerns.

• Facilitate open communication among family members involved in the senior client’s care, assisting in conflict resolution when necessary.

• Educate clients and their families about local resources, support groups, and programs available to enhance social connections and improve overall quality of life.

• Maintain detailed and accurate documentation of all client interactions, assessments, care plans, and progress, ensuring compliance with ethical best practices.
