You will operate within a climate-controlled office setting, where daily collaboration with fellow healthcare providers is encouraged. Your responsibilities will include the use of current technological tools to maintain open lines of communication and facilitate effective treatment planning.

• Conduct comprehensive assessments and evaluations of patients to determine their mental health status, utilizing appropriate diagnostic tools such as the DSM-5 to inform individualized treatment plans that are sensitive to cultural and special needs.

• Regularly monitor and assess patient progress through a variety of evidence-based interventions, adapting strategies as necessary to support the mental health diagnosis and overall well-being.

• Prepare and maintain accurate treatment records and clinical documentation on a timely basis in accordance with regulatory requirements and departmental protocols, ensuring transparency and accountability in patient care.

• Collaborate with patients, families, and interdisciplinary teams to create comprehensive treatment plans aligned with patient goals and aspirations. Actively address areas related to patient safety and symptom reduction.

• Coordinate referrals and maintain effective communication with other healthcare providers, ensuring continuity of care for patients across different service levels.
\n
• Provide education and mentorship to students and interns in social work, fostering the next generation of healthcare professionals.
\n
• Participate in peer reviews, clinical collaboration, and coaching sessions under the guidance of your clinical supervisor to enhance the development of team members.

Your primary focus will be on adults and families coping with a cancer diagnosis. You will provide them with the therapeutic support they need to manage the emotional challenges inherent in their situation. This includes addressing feelings of anxiety, depression, and grief, while also promoting resilience and well-being.

• A Master’s degree in Social Work from an accredited institution.

• Current State License as a Licensed Clinical Social Worker (LCSW) in Colorado.

• Experience in hospice care or working with oncology patients is highly desirable.

• Familiarity with evidence-based treatment modalities and their application in outpatient settings.

• Strong interpersonal skills and the ability to work effectively in a team environment.

We offer a competitive salary ranging from USD 90,000 to 105,000 annually, reflecting the advanced skills and expertise you bring to the role.
