Key Responsibilities\n

• Conduct thorough psychosocial assessments to evaluate emotional, social, and environmental strengths and challenges faced by patients and their families in relation to their health situations.

• Formulate and implement individualized care plans employing evidence-based social work interventions.
\n
• Collaborate intimately with the healthcare team, including physicians and other specialists, to ensure that the patient's needs are comprehensively addressed, involving families in all stages of care.

• Facilitate discharge planning by liaising with other healthcare providers and insurers to arrange necessary follow-up care and ensure a smooth transition from the health care facility.
\n
• Provide education and counseling to patients and families, helping them to adapt and cope with various life challenges, including illness or hospitalization.
\n
• Utilize crisis intervention techniques and provide both brief and long-term therapy, including individual, group, and family sessions.
\n
• Be available for after-hours calls as part of an on-call rotation, supporting urgent patient needs.
\n
• Advocate for patients' rights and empower them through education about their treatment and available community resources.
\n
• Act as an expert resource to fellow staff members, offering support, guidance, and education within the social work framework.

• Engage in community outreach initiatives, presenting at educational conferences to share knowledge on psychosocial oncology practices.

Patient Population / Client Focus\n

The primary focus of your role as an LCSW will be working with students and their families affected by mental health conditions, chronic illnesses, and significant life transitions. You will be instrumental in providing crucial support to high school and middle school students, ensuring they have access to the resources and assistance they need to flourish both academically and emotionally.

Qualifications\n

• A Bachelor’s Degree in Social Work is required.

• A valid LCSW license in the State of Colorado is mandatory for consideration.

Compensation and Benefits\n
