This position is primarily remote, focusing on the operational needs of our Community Health Clinic. Key Responsibilities\n

• Collaborate closely with case managers and healthcare professionals to develop and implement comprehensive discharge and treatment plans that respect and uphold patient rights.

• Facilitate ongoing professional development by supervising assigned staff, including Social Workers, MSWs, and interns, while actively participating in departmental quality improvement initiatives.

• Establish communication channels with community agencies to ensure continuity of care and resource allocation, aiming to prevent unnecessary hospital readmissions.

• Stay responsive to psychosocial service needs, ensuring timely delivery of care and adherence to discharge timelines.

• Participate in specialty team rounds and relevant patient care conferences to provide insight into patient caseloads.
