Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to determine patients' emotional, social, and financial needs.

• Collaborate closely with medical teams to develop and implement holistic care plans tailored to individual patients.

• Provide individual and group counseling to patients and their families, focusing on adjustment issues, grief, and coping mechanisms.

• Facilitate resource coordination, connecting patients and families with community organizations to arrange for post-discharge services.

• Offer emotional support and educational resources about treatment options and various support services available.

• Participate actively in hospital committees, contributing your social work expertise to interdisciplinary discussions and strategies.

• Provide crisis intervention and support during critical situations to ensure patient safety and emotional well-being.

• Document all interactions and interventions accurately to maintain compliance with hospital policies and state regulations.

This role primarily serves patients facing terminal illnesses and significant life changes, often requiring urgent emotional and psychological support. You will work with a diverse patient population, often involving their families in the therapeutic process to ensure holistic care. Qualifications\n

• Master's degree in Social Work from an accredited program.

• Active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.

Compensation and Benefits\n
